Family Law Courts have jurisdiction over all cases involving dissolution of marriage (divorce), legal separation, nullity, paternity, domestic violence, child custody, visitation and support.

Rules & Forms

Commonly used forms in Family Law proceedings including petitions for different types of restraining orders, child custody and support cases, and parentage may be found on the Judicial Council's website, this website, or at most Superior Court locations throughout Alameda County.  If you are seeking legal assistance, court staff cannot give you legal advice.  Please visit the Resources and Referrals page for more information.

About Judicial Council forms

All forms are provided in PDF format and may be viewed and printed from any computer for which Adobe Reader 6.0 or newer is installed. Forms indicated as fillable may be filled out electronically. Non-fillable versions of all forms are also available and should be completed neatly in blue or black ink. Forms may be downloaded by right-clicking on the form number link and selecting the "Save Target As..." or "Save Link As..." option to download the PDF file directly to your hard drive. Use Adobe Reader to open the file after downloading.
Please note that Domestic Violence forms are available on the Judicial Council’s website in English, Chinese, Korean, Spanish, and Vietnamese.  However, completed forms should be submitted to the Court in English.
